An elevator gets stuck between floors, and then the brake drum fails; inside, a woman has a heart attack. Despite Sharon Walters's admiration for Dr. Brackett, he sets her off, whenever he's around to see her please him, therefore, Dixie diagnoses her nursing student with Bracketitis. Later, a fireman buff develops a crush on John, who tries to get rid of her. A man is injured in his junkyard. An industrial accident takes place at a railroad yard. With the help of Dixie, and before Nurse Walters smiles in front of both Dixie and Dr. Brackett, they all take care of a man with a pulmonary embolism.

The title of this episode mentions that there's some sort of trouble in this episode, which is nothing new. The problem, or dilemma, is Dr. Bracket. But Dr. Bracket seems to think the nurse intern is the problem. She always seems to be making mistakes around ER doc, but Dixie understands the situation and knows how to fix it.

Another "dilemma" of sorts is happening at the fire station. Johnny Gage seems to have a woman after him (usually, he seems to be the one 'after' women), and he doesn't like it. He tries to get one of the other men to ask him out, and succeeds; or does he?

Meanwhile, the medical professionals do a little bit of work. They rescue a group of people from an elevator, help a man who has his own personal junk yard in his backyard, and rescues two men involved in an industrial accident. Though the most heart-warming scene is the beginning, the most humorous is the junk yard...

In the early scene in which Brackett is on the call w/ squad 51 and things are tense, we see the student nurse looking up at him worshipfully and standing about one foot away from him. Brackett moves from the EKG (?) machine back to the phone and bumps into Nurse Walters. At THAT POINT, Dixie should have stepped in and physically moved the girl away, but instead she just sat there and watched. Then Brackett bumped into the girl AGAIN, and AGAIN Dixie did nothing. She's the head nurse in the ED, so it's up to her to ensure that things run smoothly, and that includes not intentionally allowing the doctor-in-charge to get ticked off. But that's exactly what happened, and what Dixie allowed to happen. The result? Brackett lost his cool and yelled at Nurse Walters, thereby making her even more nervous and continuing the cycle of disaster.

Elsewhere, I don't know how Chet got the date with Gage's stalker Cynthia, but he was out of his mind to allow her to come into the station to meet "the guys." (And btw, I see that Johnny is already starting his habit of having some fixation to obsess about each week. I had hoped to at least get out of Season 1 before those started, but.. I guess not.) :-(
